Find your patient’s preferred DNA location or physician by visiting dneph.com/locations to find the name, address and fax number for the DNA clinic, Dallas Transplant Institute or DNA vascular access center that best suits your patient’s needs. We offer over 25 convenient locations serving the Dallas-Fort Worth Metroplex.

There are three ways to refer to DNA (electronically, by phone or by fax.)

  • ELECTRONICALLY: Request an appointment on our website or connect to us using LeadingReachLeadingReach is an online referral process that effectively facilitates a better patient handoff ensuring the continuity of care and optimal patient outcomes.
  • PHONE: Call us at 214.358.2300 or toll free at 877.654.3639, Monday through Friday, 8:30 a.m. to 5:00 p.m. CDT.  Ask for a new patient coordinator at the desired location or with the physician you wish to refer to. Our referral staff is here to work with your doctors, staff and patients to answer all inquiries and get patients scheduled in a timely manner.
  • FAX: Download our Consult Request Form. Once complete, fax the form to the preferred office’s fax number.
